Specialist Palliative Care Team

Our team visit you in your own home, or the place you call home such as a care home.

You will be contacted and visited by one of our Clinical Nurse Specialists who will arrange to assess how we can best support you. This support may be to improve complex symptoms or give guidance toward planning ahead. Your Clinical Nurse Specialist may then discharge you back to the care of your GP or district nursing team with the understanding you can receive support again should you need to. Our input depends upon your own needs. The frequency of input will depend upon this, and we will work in partnership with your GP and District Nurses to support you.

Input from your Clinical Nurse Specialist or a member of their team will be face to face, phone or can be arranged to be via video link.

Your Clinical Nurse Specialist will arrange to contact you between Monday to Friday, 8am to 6pm.
