Advance Care Planning

9:30am - Monday 14th May 2018, at Compton Hospice Education and Training Centre

Aim:

This study day will provide health and social care professionals with the necessary knowledge and skills to initiate Advance Care Planning for individuals approaching the end of life within their clinical area. You will gain knowledge relating to the legalities of Advance Care Planning and an understanding of the principles of effective communication during discussions with patients and their families.

Learning outcomes:

• Identify individuals approaching the end of life
• Demonstrate an understanding of Advance Care Planning principles and how this benefits patients and their families when facing a life limiting progressive illness.
• Initiate and reflect upon Advance Care Planning in clinical practice and the challenges

Facilitator:

• Jenny Hadley – Lecturer Practitioner in Palliative and End of Life Care. Jenny has a robust and relevant clinical background and continues to contribute to
the clinical arena at a regional level.
